The Damage Boy achievements are glitched, you only need to finish the second floor of a level to unlock it, which is why Dead Boy is glitched, Chest/Dark Room has only 1 floor. And the Samson achievement is supposed to be 2 floors in a row, any two floors, but the description says 2 levels.

Well, if anyone is interested in the story on how it was found: Spoiler Streamer found the puzzle piece by accident, you had to die in the sacrifice room (the one with spikes in the middle) while holding the Missing Poster. If you did that, the killer would show up as a puzzle piece There are 4 diferent posters, one with a diferent death, you could know which piece was from which puzzle by looking at a small mark in the puzzle piece. Reddit started to rush around finding more and more pieces, they got a lot of them and someone sugested how to unlock The Lost by doing some specific deaths. He was labed a troll. Later when the puzzle was almost finished, the streamer tried to do what the 'troll' said. He unlocked The Lost. BoI Reddit rejoiced. Thanks to Dark Bum + Soul Converter. Got a lot of soul hearts, converted them all to normal hearts, got even more soul hearts (apparently there is a limit of 12 normal hearts and 12 soul/black hearts), then I got a reroll room that gave me Ipepac + Triple Shot + Homing Tears So, there is an achievement called 'Dead Boy', seems like one of these 'Take no damage' achievements. But since there are no Cathedral/Sheol related achievements, I fear this one means 'Finish Cathedral/Sheol + Chest/Dark Room' without taking damage. Edit: The PS4 edition actually tells you what to do, its 'Beat the Chest or Dark Room without taking damage'

Its difficulty as in, if you problems with reaching and beating mom, you gonna have a bad time getting past the Womb and an even worse one killing Rebirth's heart. As for me, I couldnt find noone that got where I did, so I'm stuck here waiting for a patch.
